    "Most presidents and even most CEOs, when they make a decision that is going to have financial impact or personnel impact, you seek a lot of input," said Republican strategist Rick Tyler. "He doesn't do that. Even his own communications staff says he goes by his gut."

    The president's pronouncements and social media blasts are often surprise attacks. They can vanish as quickly as they emerge if political expedience warrants a different tack.

    His announcement last week that the GOP was going to take on health care again despite a bruising and unsuccessful effort in 2017 stunned most in his own party and seemed driven in part by a lingering desire to fulfill his oft-repeated 2016 campaign promise to repeal "Obamacare."

  38. Analyzing the Affordable Care Act

    The purpose of the Affordable Care Act (ACA) was to increase the healthcare insurance coverage and make Medicare and Medicaid more effective to Americans. To present day since implementation, the Affordable Care Act (ACA) has had some effects on the finances of healthcare organizations (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). The Act was also created with some mechanisms that were meant to reduce the negative impact of healthcare outcome due to social determinants of health such as socioeconomic statuses and access to health services among others (Billioux, Verlander, Anthony, & Alley, 2017). The 2010 Act, however, has faced great criticism especially from the Republicans dues to some of its faults. The current administration has made some changes and still proposes to make more changes in this healthcare reform (Moniz, & Gorin, 2017). Insight on the effects of ACA on US healthcare organizations, its effects on the social determinants of health, and the current administration's changes and proposed changes on the Act is essential knowledge in understanding US healthcare organizations and finance.

    Impact on healthcare fiancés

    The Affordable Care Act required that all US citizens and other legal residents should have healthcare insurance covers or else they were to face tax penalties. The Act gave an option to states to increase their Medicaid coverage to cover adults with income under 138% as per the federal poverty levels (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). 32 states tool this expansion but the other states refrained from doing so. This means that Medicaid disbursements were increased (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). This increased the number of patients who could settle their bills since initially some of the patients who were within this bracket and were not covered by Medicaid suffered from hospital bills. The states that accepted to expand their Medicaid had their healthcare organization receive more patients and higher Medicaid disbursements, increasing their revenue. However, in order to comply with Medicaid, most of these organizations were required to invest heavily in information technology to enhance their compliance with ACA. Some of the income that was obtained from these increased revenues went to heavy IT infrastructure, but still, at the end of 2013, some healthcare facilities reported high profits due to increased Medicaid disbursements (Rosko, Goddard, Al-Amin, & Tavakoli, 2018).

    Some of the organizations that registered increased profits due to the ACA at the end of 2013 include large healthcare organizations, and organizations dealing with a high number of inpatients such as nursing homes (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). Outpatient services clinics experienced little to no change in revenues and income because they did not have patients suffering from heavy bills initially as it was the case with complex hospitals dealing with complex conditions and large inpatient facilities. This high income and profits only occurred in 2013 because, in 2014, the disproportionate share of low-income patients was reduced by a 25% of the 2013 value, reducing the Medicaid disbursements to the facilities that had initially benefited (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). Additionally, many people who were newly insured with private healthcare covers had high deductibles alongside their insurance which made healthcare insurance hardly affordable. This also reduced the number of covered patients, reducing healthcare affordability and consequently the income that hospitals were to receive from healthcare insurance firms (Rosko, Goddard, Al-Amin, & Tavakoli, 2018). This meant lower income for hospitals.

    The ACA and Social Determinants of Health

    The ACA was able to close the socioeconomic gap, which was one of its aims. It increased the number of patients who were covered by Medicaid, and it also negotiated the valuation of health insurance policies that the healthcare insurance firms offered (Billioux, Verlander, Anthony, & Alley, 2017). On the healthcare access social determinant, the Affordable Care Act expanded the scope of coverage of health insurance. Insurance firms were mandated to accept and cover patients with preexisting conditions without a probation period of payment and no coverage (Billioux, Verlander, Anthony, & Alley, 2017). Additionally, the ACA was able to bring in other services that healthcare did not cover initially such as mental health and other therapies into healthcare coverage (Billioux, Verlander, Anthony, & Alley, 2017).

    When it comes to healthy behaviors and social support and coping skills, the ACA has an incentive of implementing evidence-based-models and incentivize research that could promote health proactively. The ACA also established the CMS (Center for Medicaid and Medicare Research) which was able to come up with proper information systems that were aimed at enhancing the Medicare and Medicaid payment systems and patients’ experiences (Billioux, Verlander, Anthony, & Alley, 2017). CMS recommended an Accountable Health Community (AHC) as an alternative to Medicaid and Medicare that could enhance proactive healthcare services and improve the Social Determinant of Health (Billioux, Verlander, Anthony, & Alley, 2017).

    Trump’s Administration and ACA Changes

    Initially, the Affordable Care Act was supposed to serve citizens with long-term health insurance covers. It also required that health insurance firms accept patients with preexisting conditions. These areas are currently faced by high chances of changes under the current administration (Moniz, & Gorin, 2017). The new administration has brought about short terms and associated healthcare plans that are cheaper than the long-term health insurance services that were brought about by the Affordable Care Act (ACA) (Moniz, & Gorin, 2017).

    However, the short-term care insurance plans are only cheaper to the healthy population but very costly to people with chronic illnesses (Moniz, & Gorin, 2017). The associated plan is currently under the ACA requirements to cover patients with preexisting conditions, but the executive order that seeks to loosen the health insurance rules that were initially part of the ACA might allow insurance firms to reject or change patients with preexisting conditions highly (Moniz, & Gorin, 2017). It is also expected that President Trump’s administration will also make changes to the Medicaid plan, reducing the government’s budget (Moniz, & Gorin, 2017). The ACA, as aforementioned, had increased Medicaid cover which increased revenues to big and inpatients healthcare facilities.
